There was an EN article about it at the time - he was nominated as one of the coaches' 3rd choices and therefore qualfied for the annual listing IIRC.

It was one of the FIFA reps from one of the Caribbean Islands voted for him.

As an aside, Cook was succeeded by a Scotsman, Paul Crosbie, who player-managed the nation for a few years and drafted in some Scottish mates to play for them in World Cup qualifiers.
Think they were ranked about 200th in the world at that time.
