The formal address in modern times appears to be a quirk peculiar to football.

Does anyone call someone Mr or Mrs - even the old skool offices ditched it. Quite perplexing or do loads of folks work in customer service ( “Mr Humperdink - can I call you ‘Engelbert’?”) ;)
