A back pass to the keeper;

A keeper holding on to the ball too long, or;

An indirect free kick for obstruction?

Goalies don't pick up passbacks anymore, even in panic. They are much better with their feet than ever before so that has contributed to that.

There would be about a hundred free-kicks per game if they did that.

Simon mignolet was penalised for holding onto the ball for 22 seconds in a match. My understanding is the ref only penalises a keeper if they deem it significantly beyond the 6 seconds.

i reckon most matches you could find several examples of keepers holding the ball a good bit past 6 seconds, albeit not 22!

totally agree, it’s a rule that’s been applied so rarely for such a long time that it’s basically ignored by players and refs

That was not long after the so called “8 second rule” was brought in. John Rowbotham was the ref iirc?
Cannae recall the score but pretty sure we didn’t win the game after going 1-0 in front. Sound familiar? Times never change eh?

... or the keeper kicking from hand outside the box.

how about a foul and immediate booking for a deliberate handball from an outfield player? thought that was a rule? seen plenty fouls for it but not bookings, for a long time.
