I think it was Cummings as he didn't make any movement to go and take the 4th pen but was pushed forward by Hanlon and someone else. Who was originally number 4 I do not know.

nope...he was always due to take number 4..again, confirmed in interviews after the game....

Sure the first 5 takers are pre agreed with the ref are they not, don't think you can change the order to suit?

You must give the referee the list of five penalty takers AND the order in which they will take them.
